Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay. Our goal is to treat the entire patient while improving their overall wellness and quality of life, in addition to providing free pregnancy tests, HIV/AIDS screening. At Legacy, we empower patients to lead better lives by promoting healthy behaviors and offering resources such as literacy classes, family planning services, and nutrition and weight management information.


Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.

Job Description

The Registered Nurse (RN) assumes responsibility and accountability for the delivery of patient care. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision. The nurse demonstrates critical thinking and performance ability in the coordination of patient care. Consistently performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.

Essential Functions

  • Assist Nurse Manager in supervision and education of clinic staff.
  • Assist with coordination and supervision of volunteers and students in clinic area.
  • Assist with OSHA compliance tasks and infection control.
  • Perform medical procedures and patient care according to agency protocols.
  • Administer medications according to standard protocols and physician orders.
  • Assist Nurse Manager in ongoing assessment of clinic services.
    Maintain ongoing inventory record of medications, and reorder as directed.
  • Perform phlebotomy and diagnostic tests as indicated.
  • Document client care appropriately and in a timely manner.
  • Assess, educate, and treat clients per standing or provider order.
  • Consult with clients by telephone, walk in triage, or office visit.
  • Assist providers and management as needed.
  • Assist care team in accordance with PCMH standards.
  • Assist Nurse Manager with chart audits and quality assurance as assigned.
  • Participate in Performance Improvement Program.
  • Assist with medication refills as needed.
  • Other duties as assigned.

Education & Training Requirements                                 

  • Bilingual English/Spanish preferred.
  • State of Texas Registered Nursing License.
  • CPR/BLS certified required.

Work Experience Requirements

  • 1 year clinical RN experience required.
  • 2-3 years of OB clinical experience preferred.
  • Previous community health care experience preferred.
